Job Description

Note: This position requires employees to be on-site five days a week at facilities in the Topsham and Rockland, ME area.

Note: Relocation is welcomed.

Job Description

  • Provide primary care services to elderly patients in assisted living, memory care, and long-term care settings.
  • Conduct comprehensive medical assessments, develop care plans, and manage chronic conditions.
  • Collaborate with an interdisciplinary team, including nurses, caregivers, and specialists, to ensure optimal patient care.
  • Utilize telemedicine and remote patient monitoring tools to provide continuity of care.
  • Maintain accurate and timely documentation in the Electronic Health Record (EHR) system.

Qualifications

  • Active Nurse Practitioner (NP) or Physician Assistant (PA) license in Maine.
  • Experience in geriatrics, primary care, or a related field preferred.
  • Strong ability to work independently and collaboratively within a healthcare team.
  • Proficiency with EHR systems and comfort using telehealth technology.
  • Excellent clinical assessment and diagnostic skills.
